This is where our journey together begins.

We offer a complimentary phone consultation to get to know you, your loved one, and the challenges your family is facing. We understand that the aging process — especially when chronic illness is involved — can create significant difficulties in managing care at home, and that the impact is felt by the entire family system, not just the individual.

During this initial conversation, we will listen carefully to your situation and assess whether our services are the right fit for your family’s needs. If we believe we can help, we will outline the next steps — including a comprehensive in-home assessment designed to give us a deeper understanding of your loved one’s unique circumstances and lay the foundation for a tailored care plan.

Superior Aging Care services are provided on a private pay basis. Some long-term care insurance policies may cover the cost of a comprehensive assessment — we are happy to help you explore this.

Comprehensive Assessment

The comprehensive assessment is the cornerstone of everything we do.

During an initial visit of approximately one to two hours, our Aging Life Care Manager will conduct a thorough evaluation of your loved one’s living environment alongside their cognitive, physical, and emotional health. Where cognitive concerns or suspected dementia are present, this evaluation includes validated assessment tools — such as the Montreal Cognitive Assessment (MoCA) — to better understand the level of impairment and guide the most appropriate therapeutic approach for their individual needs. More than a clinical checklist, this is an opportunity to ask the right questions — and more importantly, to truly listen for what your loved one hopes for and values as they look toward the future. 

From this assessment, we collaborate with your loved one and family to develop a personalized care plan that reflects their individual needs, goals, and aspirations. Following the assessment, you will receive a comprehensive set of recommendations and resources to guide the care process.

Should your loved one choose to move forward, we will establish an ongoing monitoring schedule — whether weekly, bi-weekly, monthly, or twice yearly — to ensure the care plan continues to evolve alongside their changing needs. During each visit, we assess progress toward established goals and adjust as needed, always with one purpose in mind: to support your loved one’s independence, dignity, and highest possible quality of life.

Advocacy

When your loved one needs someone in their corner, we are there.

As your dedicated family advocate, our primary role is to ensure that you and your family are fully informed, genuinely heard, and never left navigating the healthcare system alone. We facilitate clear, ongoing communication between families, case managers, and healthcare providers — translating complex medical information into language that makes sense and empowers confident decision-making.

During hospitalizations, we focus on what matters most: ensuring a safe, well-coordinated discharge. We work proactively with the care team to develop a comprehensive discharge plan that minimizes the risk of readmission, reduces unnecessary costs, and sets your loved one up for the smoothest possible transition home.

Beyond the hospital, our advocacy extends to the caregivers in your loved one’s life. We provide oversight to ensure they are properly trained, adequately supported, and consistently delivering the high standard of care your loved one deserves.

Because at the end of the day, having a knowledgeable, compassionate advocate by your side shouldn’t be a luxury — it should be a given.

Companionship

A good companion listens and shares when appropriate and validates the experiences of the senior. They initiate games and activities—engaging the senior in stimulating conversation, laughter, and fun. Over time this develops trust and forms a true friendship. We are happy to sit and talk, and we are also happy to accompany our clients on excursions near and far.
