Initial Consultation

This is where we begin our journey together. Through a complementary phone consultation, we take the time to learn about your loved one and the challenges your family is facing. We understand that the aging process and chronic illnesses can create significant difficulties in managing care in the home environment, impacting the entire family system. During this initial conversation, we will assess whether the situation could benefit from the support of an Aging Life Care Manager. If we find there is a good fit between our services and your needs, we will outline the next steps, which include a comprehensive home assessment. This assessment will help us gain a deeper understanding of your loved one’s unique circumstances and develop a tailored care plan that addresses their specific needs.

* Services are provided on a private pay basis. Some long-term care policies will cover the cost of a comprehensive assessment. 

Comprehensive Assessment

Our Aging Life Care Manager provides personalized care and support for your loved one during the initial visit lasting approximately one -two hours. She will conduct a comprehensive assessment of your loved one’s living environment as well as the cognitive, physical, and emotional health status. This assessment is an opportunity for us to ask clarifying questions and more importantly it allows us to listen for the client’s desires and goals for the future. Together we will collaborate to create a tailored plan of care that aligns with their individual needs and aspirations. After the assessment the Aging Life Care Manager will compile a list of recommendations for the family and/or primary caregiver which will include valuable resources to assist in the care process. If your loved one chooses to move forward with our services, the Aging Life Care Manager will establish a monitoring schedule which will include weekly, bi-weekly, monthly or twice yearly to reassess. During these visits she will monitor how the care plan is progressing and assess the attainability of established goals. If necessary, the care plan will be adjusted or modified to ensure it continues to meet your loved one’s evolving needs. Our goal as Aging Life Care Managers is to provide ongoing support and ensure that your loved one receives the highest quality of care while promoting the client’s independence and well-being.

Advocacy

As your family advocate, our primary role is to ensure that you, your family, and any representatives are fully informed about the care process. We will provide education and updates, facilitating clear communication and collaboration with case managers and health care providers. During hospitalization, our focus will be on making the discharge process as safe and smooth as possible. We will work to develop a clear and comprehensive discharge plan that outlines how your loved one can return home safely. This proactive approach to communication and care planning is essential for reducing hospital readmissions and minimizing unnecessary costs. Additionally, we will oversee the caregivers to ensure they are adequately trained and equipped to provide the highest level of care. This oversight is crucial for maintaining quality care and supporting your loved one’s needs effectively at home. Together we can create a supportive and well-coordinated care experience that promotes health and well-being for the client. 

Companionship

A good companion listens and shares when appropriate and validates the experiences of the senior. They initiate games and activities—engaging the senior in stimulating conversation, laughter, and fun. Over time this develops trust and forms a true friendship. We are happy to sit and talk, and we are also happy to accompany our clients on excursions near and far.
