UCI Health – Alzheimer's and Dementia Care (GUIDE Program)

UCI Health’s Alzheimer’s and Dementia Care program is an established participant in Medicare’s GUIDE (Guiding an Improved Dementia Experience) Model. Through this program, individuals diagnosed with dementia are paired with a dedicated care team — including a Dementia Care Specialist and Care Navigator — who conducts a comprehensive assessment and develops a personalized care plan. The program also offers caregiver education, 24/7 support access, and respite services for eligible families. This is a Medicare-covered benefit for those enrolled in traditional Medicare Parts A & B.
